Afghan Community Health Worker (CHW)

 
 

JOB SUMMARY

NEAAO is looking for a full-time community health worker to provide the following services to the Afghan community:

  • Help patients navigate health care services and other supports to reduce the risk of Covid-19. Network with systems of care to ensure the patient has access to Covid-19 vaccines, testing and treatments.

  • Keep accurate records of all referrals and interactions, and perform follow-up.

  • Participate in statewide CHW trainings and regular meetings.

  • Design outreach tools and explore opportunities to host events that support wellness and encourage proactive health care for all family members.

  • Build relationships in the community that further health equity and inspire an excellent level of health care for Afghan community members.

  • Explore and engage with childcare, food, transportation, housing, employment and income, and other resources to improve social supports for community members impacted by health concerns.

Qualifications

  • Good communication skills in English and Afghan language (Dari preferred)

  • Commitment to helping people in the community and furthering NEAAO’s mission

  • Knowledge of New Mainer community, especially those from Afghanistan

  • Driver’s license and clean driving record

  • Desire to work with families and individuals in the home and in health care settings

ADDITIONAL INFORMATION

  • Full time exempt position; state training provided

  • $20-$25 per hour depending on qualifications
